As Chief Executive Officer or Executive Director of my company/organization, I hereby request the issuance of an Aurora Public Library District Account in the name of my company/organization. I understand that according to Illinois library law, a company/organization library account may be issued only if that card is used solely for the business purposes of that company/organization. I, and the other authorized users of this card, agree to comply with this regulation. I agree to accept responsibility for all materials borrowed on my company/organization account and to pay all fines and fees which may accrue for lost or damaged materials. Only one card is to be issued and used only by authorized users. I agree to promptly notify the library in writing of any changes to the list of authorized users.
